Select the Feedback activity you have created on your Course Outline.

Select the Edit questions tab.

Use the Select item type drop-down to select a question type. The most commonly used question types are described below.

Select Label to add a heading to your survey. Enter and format the text in the HTML editor.

Select Multiple choice to add a multiple choice question to the survey. Use the question settings to adjust the layout of the answer choices and the type of multiple choice question.

Select Multiple choice (rated) to add a rated multiple choice question to the survey. Answer choices of a rated multiple choice question have numerical values associated with them, which allows students’ answer choices to be averaged. Use the question settings to adjust the layout of the answer choices and the type of multiple choice question.

Selecting Numeric answer adds a question that can only have a numeric value as an answer. Use the question settings to specify the acceptable range of answers.

Selecting Short text answer adds a question with a single line answer field. Use the question settings to specify the width of the field and the maximum number of characters that can be entered.

Selecting Longer text answer adds a question with a large text box answer field. Use the question settings to specify the width and height of the text box.

Selecting Add a pagebreak adds a page break to your survey.

After selecting a question type, enter your question in the Question field.

Select the Required checkbox if students are required to answer the question.

Adjust any settings specific to the selected question type.

By default, a question is placed at the end of the activity. If you would like to change this, use the Position drop-down to select a different position for the question.

Select Save question to add the question to the activity.

From the Preview section of the Edit questions page, you may adjust the position of questions, edit questions, change the required status of questions, or delete questions.
